1. Describe the benefiting community including its location, using any relevant geographic and demographic information (Please be as specific as possible). If the activity is a scholarship, enter "N/A". The community population of San Jose and surrounding towns located in the Central Valley of Costa Rica. There are approximately 10% of the population afflicted with facial and jaw deformities requiring the treatment by the doctors who need this specialty diagnostic and surgical corrective equipment. 2. What community needs have been identified? If the activity is a scholarship, enter "N/A". Many patients treated by the Children's Hospital who reach the age of maturity have not yet had their corrective jaw surgery and yet fall out of the system for coverage. Also, other patients afflicted with jaw and articulation diseases go undetected and suffer with misdiagnosed facial pain disorders requiring management and care that can be treated with minimally invasive surgery if detected early in the process of disease development. 3. How are these needs currently being addressed with local resources and/or government agencies, NGOs, etc.? If the activity is a scholarship, enter "N/A". Currently, despite efforts of NGO's to educated local doctors over the past several years, there are neither clinics equipped nor clinicians presently having sufficient expertise to manage the care for these patients. 1. Summarize the proposed activity(ies) in 250 words or less, including the need it will address, the intended beneficiary(ies), and the potential benefits to the community. The Central American country of Costa Rica has a significant population of people suffering with facial and jaw deformities that currently have no access for care which has led to debilitating eating and chewing disorders. These disorders will eventually lead to severe facial and articulation pain of chronic nature. Many patients afflicted with congenital facial deformities need secondary facial bone corrections as they approach their adult years, some having ankylosis of their jaw joints preventing the obtaining of proper nutrition as well as severe pain. The combined treatments via both modern minimally invasive endoscopic methods as well as through traditional surgical case can correct this progression of disease, especially if instituted early after diagnosis. This project will provide needed specialty equipment for the diagnosis and treatment of these disorders as well as will work together with the academic institution, University de Latina and local surgeons who will be able to provide this care year long. The visiting surgical project team will serve to not only educate and demonstrate all usage of the equipment on charitable demonstration surgical cases, but provide the local professionals USA certified continuing education credits. Patients afflicted with the facial pain and skeletal disorders associated with the problem will be treated both during the mission clinics as well as throughout the year by this process. 2. Describe how the benefiting community will be involved in the activity(ies) (Provide specific examples). If the activity is a scholarship, enter "N/A". The professionals affiliated with the University as well as local doctors and surgeons will all participate in the educational and provision of charitable care for this patient population. They will also work in conjunction with the visiting philanthropic international visiting surgeons during the charitable free surgeries for severe cases requiring demonstration of the use of the equipment. 4. Has the benefiting community(ies) confirmed that it would like the activity(ies) to take place? Yes, there is already an agreement between the Universidad De Latina and the Hospital Metropolitano to work in cooperation together with the Rotary Club of Tibas and the International club of Rotary Club of Carlsbad member, Dr. Jeff Moses for the coordination of the project. Dr. Moses has also been awarded a visiting professorship at the University in order to assist him in giving the continuing education credits for visiting surgeons and local professionals. The medical and dental colleges have given their approval as well. The Rotary Club of Tibas San Jose, Costa Rica will participate with the process of the grant along with Dr. Moses and the Rotarian team from District 5340. 5. Proposed Start Date: 2-FEB-2016 6. Carlsbad Club Rotarian, Dr. Jeff Moses as well as the United Services Rotarian anesthesiologist, Dr. Nick Pollard, and the Mission Bay Rotarian Operating Nurse, Hon. Patricia Hunter will all participate with this effort. 1. What will be the immediate and long-term outcomes of the activity(ies). Eventual patient access for care of facial skeletal jaw diseases including those affecting the ability to chew or use the join articulations well be made possible this project. Local clinicians will be given access to equipment and trained in its usage for the care for all patients requiring this care. 2. Explain how all involved parties will act to ensure the sustainability of the activity(ies) and/or outcomes. Since the University De Latina Dental and Medical School is involved as a partner and will host the educational symposium provided by the Pacific Clinical Research Foundation and the Smiles International Foundation USA accredited continuing education certified credits, local doctors as well as other international visiting doctors will become proficient and be able to continue the care throughout the years following the project.
